Pre-Qualification & Credit Authorization

By submitting your information, you authorize AlphaMindz, LLC, along with its representatives, successors, assignees, designees, and participating third-party funding providers ("Affiliates"), to obtain and review information reasonably necessary to evaluate your business financing request.

• Business and consumer credit reports (including hard credit inquiries, when applicable)
• Financial statements and banking information
• Verification of submitted business and personal information
• Information from creditors and financial institutions
• Any additional records reasonably required to process and underwrite your application

Privacy & Security: Your information is securely processed and used solely for evaluating your financing application. By submitting your application, you consent to AlphaMindz, LLC and its funding partners obtaining the information necessary to determine your eligibility for financing in accordance with applicable laws and our Privacy Policy.

Applicant Representations & Acknowledgments

The Merchant and each Owner and/or Officer identified in this application (each individually referred to as an "Applicant") represents, acknowledges, and agrees to the following terms and conditions by submitting this application.

Applicant Agreements & Certifications

• All information and documents provided to AlphaMindz, LLC are true, accurate, and complete.
• Applicant agrees to immediately notify AlphaMindz, LLC of any change in financial condition.
• Applicant waives and releases any claims against AlphaMindz, LLC, its Affiliates, and any information providers arising from any act or omission relating to the requesting, receiving, or release of information.
• Each Applicant represents that he or she is authorized to sign this form on behalf of the Merchant.
• Applicant agrees to receive communications from AlphaMindz, LLC or its Affiliates, including:
• Calls, emails, or mail
• Text messages/SMS from AlphaMindz, LLC upon additional opt-in
• Applicant authorizes AlphaMindz, LLC to transmit this application form, along with any information obtained in connection with this application, to any or all Affiliates for the stated purposes.
• Applicant consents to the release, by any creditor or financial institution, of any information relating to Applicant to AlphaMindz, LLC and each of its Affiliates.

Additional Authorization & Consent

• Applicant waives and releases any claims against AlphaMindz, LLC, its Affiliates, and any information providers arising from any act or omission relating to the request, receipt, verification, or disclosure of information obtained in connection with this application.
• This authorization shall remain valid until revoked by the Applicant through written notice sent to marketing@thealphamindz.com.
• Please allow up to 30 business days for your revocation request to be received and processed.

Terms of Use for Text Message (SMS)

• Your wireless carrier's standard messaging and data rates may apply to SMS correspondence. AlphaMindz, LLC does not charge for SMS content; however, you may incur charges from your wireless carrier. All such charges are billed by and payable to your wireless carrier.
• Please contact your wireless carrier for information about your messaging plan. Your carrier may impose message or charge limitations on your account that are outside of our control.
• You represent that you are the owner or authorized user of the wireless device used to receive SMS services and that you are authorized to approve any applicable charges.
• AlphaMindz, LLC may send SMS messages periodically and only with your explicit agreement as indicated by your continuation of the process flow.
• AlphaMindz, LLC is not responsible for delays or failures in receiving SMS messages, as delivery depends on effective transmission by your network operator and processing by your mobile device. SMS services are provided on an "AS IS, AS AVAILABLE" basis.
• Information obtained in connection with SMS services may include your mobile phone number, carrier name, message date and time, message content, and any other information you provide. This information may be used to contact you, provide requested services, discuss your advance, current or past due payments, and operate, develop, or improve the service.
• Your wireless carrier and other service providers may also collect data from your SMS usage, and their practices are governed by their own policies.
• AlphaMindz, LLC may disclose information when necessary to satisfy any law, regulation, governmental request, avoid liability, or protect its rights or property.
• When completing forms online or otherwise providing information in connection with the service, you agree to provide accurate, complete, and truthful information.
• The service, content, and materials received through the service are proprietary to AlphaMindz, LLC and its licensors and are intended for your personal and commercial use only. You agree not to damage, impair, interfere with, or disrupt the service or its functionality.
• AlphaMindz, LLC reserves the right to update charges and/or these Terms and Conditions from time to time. The service may be suspended, terminated, or discontinued at any time, including if you breach these terms or if your wireless service terminates or lapses.
• You agree that SMS Statement Notifications may be sent through your communication service provider. You agree to provide a valid mobile phone number for these services.
• You agree to indemnify, defend, and hold AlphaMindz, LLC harmless from and against any claims, losses, liabilities, costs, and expenses, including reasonable attorneys' fees, arising from providing a mobile phone number that is not your own or from violating applicable federal, state, or local laws, regulations, or ordinances. This obligation survives termination of the agreement.

Protection of Information

• Each Merchant, and every person signing this Application on behalf of the Merchant and/or as an Owner, authorizes AlphaMindz, LLC and its Affiliates to disclose information regarding the Merchant's, Owner's, and/or Guarantor's credit standing and business conduct to agents, affiliates, subsidiaries, and credit reporting agencies as necessary to evaluate or service this application.
• Each Merchant, Guarantor, and Owner waives, to the fullest extent permitted by applicable law, any claim for damages against AlphaMindz, LLC or its Affiliates arising from:
• Any investigation conducted by or on behalf of AlphaMindz, LLC as authorized by this Application.
• Any disclosure, verification, or exchange of information permitted under the terms of this Application.

Confidentiality

• Each Merchant acknowledges that the products, services, Application, agreements, and related documents provided by AlphaMindz, LLC (collectively, "Confidential Information") are proprietary and confidential to AlphaMindz, LLC and its Affiliates.
• Unless disclosure is required by applicable law or a valid court order, the Merchant agrees not to disclose any Confidential Information to third parties.
• Confidential Information may be shared only with individuals who have a legitimate need to know for the purpose of advising the Merchant, including:
• Attorneys
• Accountants
• Financial advisors
• Authorized employees of the Merchant
• Any advisor or authorized recipient must use the Confidential Information solely for the purpose of providing professional advice or services to the Merchant.

Privacy Policy

• AlphaMindz, LLC is committed to protecting your privacy and safeguarding the personal and business information you provide while delivering the highest level of service.
• This Privacy Policy explains how we collect, use, store, and protect the information you share with us throughout the application and funding process.
• This Policy applies only to information you voluntarily provide to AlphaMindz, LLC or information obtained from companies, financial institutions, or other parties with whom you have authorized us to conduct business.

Personal Information We Collect

• The personal information we collect about you comes from the following sources:
• Information we receive from you, such as your name, address, and telephone number, or other information that you provide to us over the phone or in documents or applications.
• Information about your transactions, such as your account balances with your creditors, payment history, account activity, and all other information that may be contained in your credit card statements or other reports relating to your debt.
• Information we receive from consumer reporting agencies and other sources, such as your credit bureau reports, collection agency reports or other communications, and other information relating to your payment histories, creditworthiness, annual income, or ability to satisfy your obligations.

We will not share your opt-in to an SMS or Email campaign with any third party for purposes unrelated to providing you with the services of that campaign. We may share your Personal Data, including your SMS opt-in or consent status, with third parties that help us provide our messaging services, including but not limited to platform providers, phone companies, and any other vendors who assist us in the delivery of text messages.
• We may disclose such information in order to effect or carry out any transaction that you have requested of us or as necessary to complete our contractual obligations with you. We may also share your information with service providers that perform business operations for us, companies that act on our behalf to market our services, or others only as permitted or required by law, such as to protect against fraud or in response to a subpoena.
